REQUIREMENTS

1. Filled-out Request for Certification of Passport
2. Clear copy of the passport’s data page (a clear copy of the data page of both old and current passports are required in case of certification that one passport is a renewal of the other usually required by banks in China for updating of records)
3. Fee of RMB 190.00
4. For minor applicants, their parent or legal guardian may submit the application on their behalf. Attach proof of filiation (for parents) or guardianship (for legal guardians), along with identification documents, to the Request for Certification of Passport. 
5. For married women, attach a copy of marriage certificate if the recent passport reflects the husband’s surname.

Note: Payment may be made in cash or through bank transfer.
