REQUIREMENTS


For the shipment of human remains:

  1. Four (4) original copies of the duly-accomplished Report of Death form;
  2. Notarized Death Certificate from the hospital;
  3. Mortuary certificate issued by the Philippine Consulate (separate fee of RMB 190.00);
  4. Permit for exportation of coffin with corpse;
  5. Itinerary (complete flight details);
  6. Name and contact details of consignee in the Philippines;
  7. Name and contact details of consignor in China;
  8. Passport of the deceased;
  9. Fee: RMB190.00
  10. Notarized affidavit of delayed registration of deceased who died more than one year before the report. Fee RMB 190.00

In cases where the deceased’s body is cremated and the ashes are to be shipped in an urn:

  1. Four (4) original copies of the duly-accomplished Report of Death form;
  2. Notarized Death Certificate from the hospital;
  3. Cremation Certificate;
  4. Itinerary (complete flight details);
  5. Passport of the deceased; and
  6. Fee: RMB190.00

Note: Payment may be made in cash or through bank transfer.
