REQUIREMENTS – CONSULAR SERVICES
REPORT OF BIRTH
- Personal appearance of at least one parent (if one of the parents is not a Filipino citizen, the Filipino parent must personally appear and report the child’s birth);
- Four (4) original copies of the duly-accomplished Report of Birth form;
- One (1) original and four (4) photocopies of Birth Certificate (obtained from the hospital where the child was born) duly authenticated by Foreign Affairs Office (FAO)
- FAO-authenticated birth certificates should likewise be authenticated by the Philippine Consulate. Authentication fee is RMB190.00. For the Consulate’s authentication, the birth certificate should contain:
- the local Notary Public Office’s (with English translation) notarization; and
- the FAO’s authentication (referring to the Foreign Affairs Office of Chongqing, Yunnan, and Guizhou);
- One (1) original and four (4) photocopies of the duly authenticated Marriage Certificate of the parents or their Report of Marriage:
- Four (4) copies of the data page of the parents’ passports;
- If the marriage was solemnized in the Philippines, the marriage certificate must be from the Philippine Statistics Authority and authenticated by the Department of Foreign Affairs; and
- If the marriage was solemnized in China or another country, the Report of Marriage filed with the Philippine Embassy or any of the Consulate General must be submitted.
- Fee: RMB190.00
- Notarized affidavit of delayed registration for children who were born more than one (1) year before the report. Fee RMB 190.00
REPORT OF MARRIAGE
- Four (4) original copies of the duly-accomplished Report of Marriage form;
- One (1) original and four (4) photocopies of the authenticated Marriage Certificate. The marriage certificate (obtained from the local Marriage Bureau where the marriage was performed) must be duly authenticated by the Foreign Affairs Office (FAO).
- FAO-authenticated marriage certificates should likewise be authenticated by the Philippine Consulate General. Authentication fee is RMB190.00. For the Consulate’s authentication, the marriage certificate should contain:
- the local Notary Public Office’s (with English translation) notarization; and
- the FAO’s authentication (referring to the Foreign Affairs Office of Chongqing, Yunnan, and Guizhou);
- Four (4) copies of the data page of the applicants’ passports;
- Fee: RMB190.00
- Notarized affidavit of delayed registration for applicants who were married for more than one (1) year before the report: Fee RMB 190.00
REPORT OF DEATH
For the shipment of human remains:
- Four (4) original copies of the duly-accomplished Report of Death form;
- Notarized Death Certificate from the hospital;
- Mortuary certificate issued by the Philippine Consulate (separate fee of RMB 190.00);
- Permit for exportation of coffin with corpse;
- Itinerary (complete flight details);
- Name and contact details of consignee in the Philippines;
- Name and contact details of consignor in China;
- Passport of the deceased;
- Fee: RMB190.00
- Notarized affidavit of delayed registration of deceased who died more than one year before the report. Fee RMB 190.00
In cases where the deceased’s body is cremated and the ashes are to be shipped in an urn:
- Four (4) original copies of the duly-accomplished Report of Death form;
- Notarized Death Certificate from the hospital;
- Cremation Certificate;
- Itinerary (complete flight details);
- Passport of the deceased; and
- Fee: RMB190.00
SOLEMNIZATION OF MARRIAGE AT THE CONSULATE GENERAL FOR FILIPINO COUPLE
Step 1: File an Application for Marriage and Secure a Marriage License
- Both parties shall file two (2) sets of original and one (1) set of photocopy of application for a marriage license at the Philippine Consulate General (download or obtain copy from the Consulate), with the following attachments listed below:
(a) Passport data page;
(b) Copy of authenticated birth certificate;
(c) Copy of authenticated Certificate of Non-Availability of Marriage;
(d) 4 pieces of latest 2x2 photo (1 photo shall be attached to each of the original application form while the remaining 2 will have to be submitted along with the application form);
(e) Copy of authenticated parental consent if applicant is 18-21 years old or copy of authenticated parental advice if applicant is 21-25 years old;
(g) Other additional documents as may be required by the Consulate General upon evaluation of the application.
- Fee per application is RMB 190.00 or a total of RMB 380.00 for both parties.
- The Consulate General will publish a notice of the application for marriage license for 10 consecutive days.
- The marriage license will be issued to the applicants after 10 days of publication.
- The marriage license shall be valid for 120 days from the date of issuance.
Step 2. Actual Solemnization of Marriage
- During the validity of the marriage license, the parties together with their witnesses (at least two witnesses of legal age) shall personally appear before the officiating officer at the Consulate General on the scheduled date and time of the ceremony.
- The parties and the witnesses shall sign the Certificate of Marriage (prepared by the Consulate)
- Fee is RMB 456.00
- The parties shall also file their Report of Marriage thereafter (please see requirements above for Report of Marriage)
Documents to bring for the solemnization of marriage:
- Original and four (4) copies of the Marriage License
- Original and four (4) copies of authenticated PSA-issued Birth Certificate of each party. The Birth Certificate must have been authenticated by the Department of Foreign Affairs
- Original and four (4) copies of PSA-issued Certificate of Non-Availability of Marriage (CENOMAR) of each party. The CENOMAR must have been authenticated by the Department of Foreign Affairs
- Original and four (4) copies of authenticated Parental Consent (if the applicant is from 18-20 years old), or original and four (4) copies of authenticated Parental Advice (if the applicant is from 21-25 years old). The Parental Consent or the Parental Advice must be duly notarized, submitted to the Regional Trial Court for certification, and then authenticated by the Department of Foreign Affairs
- Four copies of passport data page of each party (original passport must be brought for inspection)
- Baptismal Certificate and Transcript of Record (School Record) or School Diploma (If applicant has no passport or expired/invalid passport and/or if there are discrepancies with respect to the date of birth and/or the name in the certified documents such as the passport, Birth Certificate, etc.).
Additional requirements, if applicable:
- Court Order and Certificate of Finality - If either or both parties had a previous marriage which was annulled or declared void by the court, such party or parties must secure a copy of said order and have the same authenticated by the DFA.
- PSA-issued Marriage Contract of the previous marriage with annotation of the fact of annulment or declaration of nullity mut be submitted.
- If an applicant is a widow/widower, a copy of DFA-authenticated PSA-issued Death Certificate of the former spouse must likewise be presented.
Once these documents are obtained, submit these together with the following:
- Duly Accomplished Form No. O5 for Legal Capacity to Contract Marriage (Forms available at the Consulate)
- Passport (2) Copies of Data page / Picture page. (Both Parties)
- Fee: RMB 190.00 each
- PROCESSING TIME: THREE (3) TO FIVE (5) WORKING DAYS
CERTIFICATE OF LEGAL CAPACITY TO CONTRACT MARRIAGE (LCCM)
- Duly accomplished LCCM form;
- Personal appearance;
- Original and one (1) photocopy of PSA-issued Birth Certificate duly authenticated by the Department of Foreign Affairs;
- Original and one (1) photocopy of PSA-issued Certificate of Non-Availability of Record of Marriage (CENOMAR) duly authenticated by the Department of Foreign Affairs;
- Original passport and a photocopy of the data/picture page of the passport; and
- Fee: RMB 190.00
- Number of Processing Days - Three (3) working days
- Expedite fee for one (1) day release - RMB 76.00
- Additional Requirements: Authenticated Parental Consent if the applicant is 18-21 years old or Authenticated Parental Advice if the applicant is 21-25 years old.
AFFIDAVITS, SWORN STATEMENTS, CERTIFICATION
- Personal appearance;
- Original passport of the applicant and photocopy of the passport’s data and visa pages;
- Original and photocopy of the document to be notarized; and
- Fee: RMB190.00
- Number of Processing Days - Three (3) working days
- Expedite fee for one (1) day release - RMB 76.00
ACKNOWLEDGMENT
- Personal appearance;
- Original passport of the applicant and photocopy of the passport’s data and visa pages;
- Original and photocopy of the document to be notarized;
- Photocopy of the passport’s data page of both parties; and
- Fee: RMB190.00
- Number of Processing Days - Three (3) working days
- Expedite fee for one (1) day release - RMB 76.00
Additional requirements for the acknowledgement of contract of employment:
- Personal appearance of either one or both of the parties who signed the contract;
- Original passport of the party who apply for the acknowledgement of contract and photocopy of the passport’s data page;
- Original and photocopy of the contract to be notarized, both parties should have signed the contract, which must bear the company seal;
- Copy of the registration of the company; and
- Photocopy of the data page of both parties’/signatories’ passports.
- Fee: RMB190.00
If the signatory for the employer is an authorized representative, the submission must include a duly executed authorization from the President or Chairperson of the company clearly specifying the (a) name of the authorized signatory, (b) in what capacity he or she is signing on behalf of the company, and (c) the delegation of the authority to sign on behalf of the company/ President/ Chairperson.
The authorization must contain an attachment of the passport data page/ national ID of the President or Chairperson delegating such authority to sign.
The employment contract should contain the minimum standards required under the POEA M.C. 08, series of 2018.
TRAVEL TAX EXEMPTION CERTIFICATE
- Application form;
- Photocopy of Philippine passport with latest Chinese visa;
- Photocopy of residence permit in China or Certificate of Employment from employer.
AUTHENTICATION OF DOCUMENTS
- For documents issued by the Chinese Government (Chongqing, Yunnan, and Guizhou):
- Original and (1) photocopy of the authenticated document:
-Authentication of Chinese documents require prior notarization by a notary public in China and authentication by the Foreign Affairs Office of Chongqing, Yunnan, or Guizhou.
- Fee: RMB 190.00
- For documents issued in the Philippines to be used in Chongqing, China (under special circumstances and when required only by the Chinese government agency to whom the document will be submitted):
- Original document authenticated by authorized authentication offices in the Philippines (issuing agency and DFA);
- Photocopy of the document;
- Original passport and one (1) photocopy of the passport data page of the applicants; and
- Fee: RMB 190.00
