- Personal appearance
- Accomplished application form and appointment through www.passport.gov.ph
- Original copy of Philippine Statistics Authority (PSA formerly NSO) Authenticated Birth Certificate
- Married Females (who are using their spouse's last name) must also submit the original PSA Authenticated Marriage Contract or Report of Marriage
- Any of the following Valid IDs with one (1) Photocopy:
- Social Security System (SSS)/ Government Service Insurance System(GSIS) Unified Multi-Purpose Identification (UMID)Card
- Land Transportation Office (LTO) Driver’s License
- Professional Regulatory Commission (PRC) ID
- Overseas Workers Welfare Administration (OWWA) / Integrated Department of Labor and Employment (iDOLE) card
- Commission on Elections (COMELEC) Voter’s ID or Voter’s Registration Record from COMELEC Head or Regional Office
- Philippine National Police (PNP) Firearms License
- Senior Citizen ID
- Persons with Disabilities (PWD) ID
- School ID (for students)
- For applicants based overseas, they may use their host government issued ID's showing their Philippine citizenship. (Example: Residence Card)
- Other Supporting documents will be further required in the following cases: a) late registered birth certificate; b) absence of birth certificate or report of birth; c) dissolution of marriage and the applicant wants to revert to her maiden surname; d) incomplete data in the birth certificate or report of birth; e) has discrepancy in data in birth ceriticate or report of birth; f) dual citizen; g) naturalized Filipino citizen; h) acquisition of Filipino citizenship by election; and h) grant of Filipino citizenship by act of legislation.

- If applicable: Original PSA authenticated documents (birth certificate and marriage contract) that will support the change of name: a) renewal applications for lost and mutilated passports; b) renewal applications requiring changes in the passport entries; c) renewal applications of old brown and green passports bearing no complete middle name; and d) applicants included in the Department's watchlist.

- Accomplished application form and appointment through www.passport.gov.ph
- Personal appearance of minor applicant and either parent or authorized adult companion
- Current ePassport with photocopy of data page
- Marriage Certificate is required if only one parent is accompanying the child
- If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child
- If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
- Original and photocopy of proof of filiation and/or guardianship
- Passport or Valid Government issued ID of either parent with one (1) photocopy (click here for the List of Acceptable IDs for Passport Application)
