REQUIREMENTS – CONSULAR SERVICES

 

REPORT OF BIRTH

  1. Personal appearance of at least one parent;
  2. Four (4) original copies of the duly-accomplished Report of Birth form;
  3. One (1) original and four (4) photocopies of Birth Certificate (obtained from the hospital where the child was born) duly authenticated by Foreign Affairs Office (FAO)
  4. FAO authenticated birth certificates should likewise be authenticated by the Philippine Consulate. Authentication fee is RMB190.00;
    1. the local Notary Public Office (with English translation)
    2. the Foreign Affairs Office (FAO) of Chongqing (or Yunnan/Guizhou);
  5. One (1) original and four (4) photocopies of the Marriage Contract of the parents duly authenticated:
  6. Four (4) copies of the data page of the parents’ passports;
    1. If the marriage was solemnized in the Philippines, the marriage contract must be duly authenticated by the National Statistics Office (NSO) and by the Department of Foreign Affairs (DFA) Authentication Division.
    2. If the marriage was solemnized in China or other country, Report of Marriage filed with the Philippine Embassy or Consulate General;
  7. Fee: RMB190.00

 REPORT OF MARRIAGE

  1. Four (4) original copies of the duly-accomplished Report of Marriage form;
  2. One (1) original and four (4) photocopies of the authenticated Marriage Contract. The marriage contract (obtained from the local Marriage Bureau where the marriage was performed) duly authenticated by FAO.
  3. FAO-authenticated marriage contracts should likewise be authenticated by the Philippine Consulate. Authentication fee is RMB190.00;
    1. the local Notary Public Office (with English translation)
    2. the Foreign Affairs Office (FAO) of Chongqing (or Yunnan/Guizhou)
  4. Four (4) copies of the data page of their passports;
  5. Fee: RMB190.00

 REPORT OF DEATH

For the shipment of human remains:

  1. Four (4) original copies of the duly-accomplished Report of Death form;
  2. Notarized Death Certificate from the hospital;
  3. Mortuary certificate issued by the Philippine Consulate with fee of RMB 190.00;
  4. Permit for exportation of coffin with corpse;
  5. Itinerary (complete flight details);
  6. Name and contact details of consignee in the Philippines;
  7. Name and contact details of consignor in China;
  8. Passport of the deceased; and
  9. Fee: RMB190.00

In cases where the deceased’s body is cremated and the ashes to be shipped in an urn:

  1. Four (4) original copies of the duly-accomplished Report of Death form;
  2. Notarized Death Certificate from the hospital;
  3. Cremation Certificate;
  4. Itinerary (complete flight details);
  5. Passport of the deceased; and
  6. Fee: RMB190.00

SOLEMNIZATION OF MARRIAGE AT THE CONSULATE GENERAL FOR FILIPINO COUPLE

 Step 1: File an Application for Marriage and Secure a Marriage License

  1. Both parties shall file two (2) sets of original and one (1) set of photocopy of application for a marriage license at the Philippine Consulate General (download or obtain copy from the Consulate), with the following attachments listed below:

(a) Passport data page;

(b) Copy of authenticated birth certificate;

(c) Copy of authenticated CENOMAR

(d) 4 pieces of latest 2x2 photo (1 photo shall be attached to each of the original application form while the remaining 2 will have to be submitted along with the application form);

(e) Copy of authenticated parental consent if applicant is 18-21 years old;

(f) Copy of authenticated parental advice if applicant is 21-25 years old;

(g) Other additional documents as may be required by the Consulate General upon evaluation of the application.

  1. Fee per application is RMB 190.00 or a total of RMB 380.00 for both parties.
  1. The Consulate General will publish a notice of the application for marriage license for 10 consecutive days.
  1. The marriage license will be issued to the applicants after 10 days of publication.
  1. The marriage license shall be issued thereafter and will be valid for 120 days from the date of issuance.

Step 2. Actual Solemnization of Marriage

  1. During the validity of the marriage license, the parties together with their witnesses (at least two witnesses of legal age) shall personally appear before the officiating officer at the Consulate General on the scheduled date of the wedding.
  1. The parties and the witnesses shall sign the Certificate of Marriage (prepared by the Consulate)
  1. Fee is RMB 456.00

Documents to bring for the solemnization of marriage:

  1. Original and four (4) copies of the Marriage License
  1. Original and four (4) copies of Authenticated Birth Certificate of each party, which has to pass through A. Philippine Statistics Authority, B. Department of Foreign Affairs (Authentication Section),
  1. Original and four (4) copies of Authenticated Certificate of Non-Availability of Record of Marriage (CENOMAR) of each party, which has to pass through A. Philippine Statistics Authority,  B. Department of Foreign Affairs (Authentication Section)
  1. Original and four (4) copies of Authenticated Parental Consent (if an applicant is from 18-20 years old), or Original and four (4) copies of Authenticated Parental Advice (if an applicant is from 21-25 years old) which has to pass through : A. Notary Public,  B. Regional Trial Court, C. Department of Foreign Affairs (Authentication Section)
  1. Four copies of passport data page of each party (originally passport must be brought for inspection)
  1. Baptismal Certificate and Transcript of Record (School Record) or School Diploma (If applicant has no passport or expired/invalid passport or/and if there are discrepancies on the date of birth or/and name in the certified documents such as the passport, Birth Certificate, etc.).

Additional Requirements, if applicable: 

  1. Court Order and Certificate of Finality - If either or both parties has/have married before and the marriage was annulled or declared null and void, such party or parties must secure a court order annulling or declaring the previous marriage as null and void and such order must be duly authenticated by DFA. 
  1. PSA-NSO Marriage contract of previous marriage with annotation on annulment.
  1. If widow/widower, copy of authenticated PSA-NSO Death Certificate.

Once these documents are obtained, submit these together with the following:

  1. Duly Accomplished Form No. O5 for Legal Capacity to Contract Marriage (Forms available at the Consulate)
  1. Passport (2) Copies of Data page / Picture page. (Both Parties) 
  1. Fee: RMB 190.00 each
  1. PROCESSING TIME: THREE (3) TO FIVE (5) WORKING DAYS 

 CERTIFICATE OF LEGAL CAPACITY TO CONTRACT MARRIAGE (LCCM)

  1. Duly accomplished LCCM form;
  2. Personal Appearance;
  3. Original and one (1) photocopy of National Statistics Office (NSO)-issued Birth Certificate (BC) and duly authenticated by the Department of Foreign Affairs (DFA);
  4. Original and one (1) photocopy of National Statistics Office (NSO)-issued Certificate of Non-Availability of Record of Marriage (CNRM) and duly authenticated by the Department of Foreign Affairs (DFA);
  5. Original passport and a photocopy of the data/picture page of the passport; and
  6. Fee: RMB 190.00
  7. Number of Processing Days - Three (3) working days
  8. Expedite fee for one (1) day release - RMB 76.00
  9. Additional Requirements: Authenticated Parental Consent for (18-21 years old); or Authenticated Parental Advice (for 21-25 years old)
AFFIDAVITS, SWORN STATEMENTS, CERTIFICATION
  1. Personal appearance;
  2. Original passport of the applicant and photocopy of the passport’s data and visa pages;
  3. Original and photocopy of the document to be notarized; and
  4. Fee: RMB190.00
  5. Number of Processing Days - Three (3) working days
  6. Expedite fee for one (1) day release - RMB 76.00

 ACKNOWLEDGMENT 

  1. Personal appearance;
  2. Original passport of the applicant and photocopy of the passport’s data and visa pages;
  3. Original and photocopy of the document to be notarized;
  4. Photocopy of the passport’s data page of both parties; and
  5. Fee: RMB190.00
  6. Number of Processing Days - Three (3) working days
  7. Expedite fee for one (1) day release - RMB 76.00 

Please note of the following requirements for the acknowledgement of contract of employment.

  1. Personal appearance of either of the party who signed the contract;
  2. Original passport of the party who apply for the acknowledgement of contract and photocopy of the passport’s data page 
  3. Original and photocopy of the contract to be notarized, both parties should signed the contract and with company seal;
  4. Copy of the registration of the company; and
  5. Photocopy of the passport’s data page of both parties/signatories
  6. Consular Fee: RMB190.00 
  7. If the signatory for the employer is an authorized representative, the submission must include a duly executed authorization from the President or Chairperson of the company clearly specifying the (a) name of the authorized signatory, (b) in what capacity he or she is signing on behalf of the company, and (c) the delegation of the authority to sign on behalf of the company/President/Chairperson;
  8. The authorization must contain an attachment of the passport data page/national ID of the President or Chairperson delegating the authority to sign.

It should have basic provisions as stipulated in the attached POEA M.C. 08, series of 2018.

TRAVEL TAX EXEMPTION CERTIFICATE

  1. Application form;
  2. Photocopy of Philippine passport with latest Chinese visa;
  3. Photocopy of residence permit in China or Certificate of Employment from employer. 

AUTHENTICATION OF DOCUMENTS

Documents issued in the Philippines to be used in Chongqing, China

  1. Original document authenticated by authorized authentication offices in the Philippines (issuing agency and DFA);
  2. Photocopy of the document;
  3. Original passport and one (1) photocopy of the passport data page; and
  4. Fee: RMB 190.00
  5. Number of Processing Days - Three (3) Working days

Documents issued by the Chinese Government:

1. Original and (1) photocopy of the authenticated document:

-Authentication of Chinese documents require prior notarization by a notary public in China and authentication by the Foreign Affairs Office in Chongqing (or Yunnan/Guizhou); and

2. Fee: RMB 190.00

CERTIFICATION FOR CAPTURING OF FINGERPRINTS FOR THE NBI APPLICATIONS

  1. Personal Appearance
  2. Duly accomplished NBI Form
  3. Original Passport and one (1)photocopy
  4. Two pieces of photo (2 x 2, white background)
