  1. REQUIREMENTS FOR new passport by ADULT APPLICANTs:
  • Personal appearance
  • Original copy of Philippine Statistics Authority (PSA formerly NSO) Authenticated Birth Certificate
  • Married Females (who are using their spouse's last name) must also submit the original PSA Authenticated Marriage Contract or Report of Marriage
  • Any of the following Valid IDs with one (1) Photocopy:
    • Social Security System (SSS)/ Government Service Insurance System(GSIS) Unified Multi-Purpose Identification (UMID)Card
    • Land Transportation Office (LTO) Driver’s License
    • Professional Regulatory Commission (PRC) ID
    • Overseas Workers Welfare Administration (OWWA) / Integrated Department of Labor and Employment (iDOLE) card
    • Commission on Elections (COMELEC) Voter’s ID or Voter’s Registration Record from COMELEC Head or Regional Office
    • Philippine National Police (PNP) Firearms License
    • Senior Citizen ID
    • Persons with Disabilities (PWD) ID
    • School ID (for students)
    • For applicants based overseas, they may use their host government issued ID's showing their Philippine citizenship. (Example: Residence Card)
  • Other Supporting documents will be further required in the following cases: a) late registered birth certificate; b) absence of birth certificate or report of birth; c) dissolution of marriage and the applicant wants to revert to her maiden surname; d) incomplete data in the birth certificate or report of birth; e) has discrepancy in data in birth ceriticate or report of birth; f) dual citizen; g) naturalized Filipino citizen; h) acquisition of Filipino citizenship by election; and h) grant of Filipino citizenship by act of legislation.

  • Personal Appearance
  • Accomplished application form and appointment through www.passport.gov.ph
  • Current e-Passport with photocopy of data page
  • Passport Fee:  RMB 456.00
  • If applicable: Original PSA authenticated documents (birth certificate and marriage contract) that will support the change of name: a) renewal applications for lost and mutilated passports; b) renewal applications requiring changes in the passport entries; c) renewal applications of old brown and green passports bearing no complete middle name; and d) applicants included in the Department's watchlist.
  1. REPLACEMENT OF LOST VALID PASSPORT
  • Affidavit of Loss
  • Police Report
  • Original copy of authenticated Philippine Statistics Authority birth certifiate 
  • Proof of identity / Supporting documents indicating full name, date and place of birth and citizenship
  • Passport Fee including penalty  - RMB1,140.00

  • Accomplished application form and appointment through www.passport.gov.ph
  • Personal appearance of minor applicant and either parent or authorized adult companion (with SPA in the case of the latter)
  • Original PSA-authenticated Birth Certificate or Report of Birth
  • Original PSA-authenticated Marriage Certificate of parents or Report of Marriage
  • Passport or valid government-issued ID of either parent or accompanying adult
  • School ID (if available)
  • Passport Fee:  RMB 456.00
  1. REQUIREMENTS FOR PASSPORT RENEWAL BY MINOR APPLICANTS
  • Accomplished application form and appointment through www.passport.gov.ph
  • Personal appearance of minor applicant and either parent or authorized adult companion
  • Current ePassport with photocopy of data page
  • Marriage Certificate is required if only one parent is accompanying the child
  • If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child
  • If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
    • Original and photocopy of proof of filiation and/or guardianship
    • Passport or Valid Government issued ID of either parent with one (1) photocopy (click here for the List of Acceptable IDs for Passport Application)
    • School ID (if applicable)
  • Passport Fee:  RMB 456.00

The Consulate General forwards the ePassport computer-generated forms to the Office of Consular Affairs, DFA. Processing time will take 6-8 weeks from the time the application is submitted to the DFA.
